Our nurses conduct a comprehensive, individualised assessment covering continence, cognitive function and falls risk where relevant. This includes a clinical review, a discussion of your daily routine and goals, and practical recommendations. You’ll receive a detailed written report that can support your NDIS plan, aged care needs, or general wellbeing.

No referral is required. You can self-refer directly through our online Referral Form or by contacting us. We also welcome referrals from support coordinators, GPs, aged care providers and family members.

Yes. While we specialise in supporting aged care and NDIS clients, we also welcome privately-funded clients. Simply contact us and we’ll talk you through the process and costs involved.

Most appointments take around an hour, whether in person or via telehealth. Following the appointment, our nurses prepare a detailed written report with findings and recommendations, which is typically provided within a few business days.

We provide telehealth nursing assessments Australia-wide, with a strong focus on Brisbane, Sydney, Melbourne, Perth, Canberra, Adelaide, Darwin and Hobart. Telehealth means you can access our services no matter where you live, including regional and remote areas.

Yes. All of our nurses are AHPRA-registered and hold current police checks, CPR and First Aid certifications, so you can feel confident you’re receiving safe, professional and compassionate care.

Yes. Alongside continence assessments, we offer dedicated falls risk and cognitive assessments, and can combine these where appropriate to give a fuller picture of a client’s needs and support planning.

You can submit a referral directly through our online Referral Form, or get in touch via phone or email. We work closely with support coordinators and aged care teams to make the referral process quick and straightforward.

You’ll receive a comprehensive written report outlining our clinical findings, recommendations and an action plan. This report can support NDIS plan reviews and funding discussions, aged care needs, or simply be shared with your broader care team.

While most of our assessments are delivered via telehealth Australia-wide, we also offer face-to-face nursing assessments in Brisbane and the Gold Coast. If you’d prefer an in-person visit, let us know when booking and we’ll confirm availability in your area.

Yes, supporting aged care clients at home is a core part of what we do. Our nurses can visit you in your own home (in Brisbane and the Gold Coast) or connect via telehealth, to help you stay safe, independent and well supported in the environment you know best.

For privately-funded clients we offer a flexible, tailored service that doesn’t require a formal written report. Instead, you can book a one-hour consultation where our nurse gets to know your needs, recommends suitable continence or care products, and reviews any products you’re already using to make sure they’re the right fit. This service is billed hourly, so you only pay for the time you use — simply contact us to arrange a session.
