All assessments are delivered via telehealth (video or phone), with structured questionnaires, guided discussion, and collaborative goal setting. Each service includes a timed assessment, review of relevant history, functional and risk considerations, tailored recommendations, and a written report that can be shared with support coordinators, care providers, or families. Follow-up clarification support is also available where required.

 

CONTINENCE ASSESSMENT FOR AGED CARE

Supporting Independence and dignity with personalised continence assessments. We organise free trials of continence products and organise external funding such as MASS/CAPS to assist with purchasing clients continence aids.

Clients receive a personalised management plan, product trial guidance, and assistance accessing external funding such as MASS or CAPS. This service forms part of our broader continence and nursing services delivered through telehealth nationwide.

 

 

COGNITIVE ASSESSMENTS

Our cognitive registered nurse assessment services include structured cognitive screening using PAS or RUDAS tools, behavioural observations, and evaluation of memory, orientation, and daily living function. Consultations are completed via secure telehealth and generally take 60-90 minutes, with time allocated to discuss concerns with the client and carer.

 The final report includes screening results, behaviour management strategies, risk and safety considerations, and recommendations to support care planning and goal achievement. This service supports both aged-care and NDIS contexts.

 

 

FALLS RISK ASSESSMENTS

Our fall-risk online nursing services assess mobility, balance, environmental risks, medication factors, and previous fall history to identify the likelihood of falls and related harm. The assessment is completed through a structured telehealth consultation of approximately 60-90 minutes, supported by guided discussion of the home environment and daily routines.

Clients receive a practical action plan, targeted recommendations, and strategies to enhance safety while maintaining independence. Ongoing support needs can also be reflected in the final assessment report.
